Intracellular
macromolecules are released from dying epithelial cells and may be
detected in patient blood. Keratins (Cytokeratins) are
suitable biomarkers for epithelial cell death since
these proteins are only expressed in this cell type. Keratin 18 (K18,
Cytokeratin 18 or CK18) is cleaved by caspases during apoptosis – the
presence of caspase-cleaved K18 in plasma therefore shows
epithelial cell apoptosis. In analogy, the presence of only
uncleaved K18 in plasma indicates epithelial cell necrosis.

Interpretation of M30/M65 ratios
Death mode can be determined by measuring "M30" and
"M65" values and by calculating a ratio. However, such
experiments must be carefully controlled. CK18 is
generally not completely cleaved by caspases during
apoptosis. The extent of cleavage varies between
different cell systems and can be as low as 20-25% after
treatment with an apoptotic stimulus.
It is advised that epithelial/carcinoma cells are
treated with an apoptotic agent (e.g. staurosporine) and
a necrotic agent (e.g. H2O2) to provide reference values
for the agent that is going to be tested.
Interpreting "M30/M65 ratios" in situations where no
references can be established is difficult (i.e. in
serum samples from patients) and ratios between
caspase-cleaved to total CK18 must therefore be
interpreted with caution.
IMPORTANT: it is not appropriate to interpret a ratio of
"M30" to "M65" of 0.3 in a serum sample as 30% apoptosis
and 70% necrosis. An almost total absence of
caspase-cleaved CK18 (i.e. a ratio of < 5%) in a serum
sample containing high levels of CK18 indicates a major
component of necrotic cell death; ratios of > 20%
indicate apoptosis.

M30-Apoptosense® is an
enzyme-linked immunosorbent assay (ELISA) for the quantitative detection
of caspase-cleaved keratin 18 (ccK18, caspase-cleaved Cytokeratin 18,
ccCK18, CK18Asp396-NE or M30 neo-epitope) levels in cytosolic cell
extracts, cell culture supernatants, human serum or plasma or other body
fluids. The M30 Apoptosense® ELISA uses the M30 antibody which is
specific to this K18 cleavage site and does not react with viable or
necrotic cells (Leers et al., 1999).
The M30 Apoptosense® ELISA measures the accumulation of a
caspase-generated product resulting in an unmatched sensitivity thus
allowing the tracing of apoptotic cells of epithelial origin in patient
plasma/serum. The most important applications are cancer (Kramer et al.,
2004; Kramer et al., 2006) and liver disease (Bantel et al. 2004;
Wieckowska et al. 2006).
The assay is ideally suited for high throughput screening for anticancer
drugs (Hägg et al. 2002; Erdal et al. 2005).
M30 Apoptosense® ELISA is
a unique tool for reliable and reproducible quantitative detection of
apoptosis of keratin 18 (K18) positive cells. The assay is intended for
patient sera. Another useful application is the detection of tumor
apoptosis in mouse xenograft models using plasma samples.
M30 Apoptosense® is an ELISA assay based on the M30 monoclonal antibody
(Leers
et al.,
1999). This antibody specifically
binds caspase-cleaved keratin 18 (ccK18, caspase-celaved cytokeratin 18,
ccCK18 or K18-Asp396) but does not bind native, uncleaved, K18. The
M30 Apoptosense® assay therefore represents a specific quantitative test
for apoptosis in epithelial cells (Hägg
et al.
2002).

separable strips provide the flexibility for your experimental design.
Monitoring cancer treatment
The most frequent forms of cancer in humans are
the lung, breast, prostate and colon carcinomas. These tumors all arise
from simple epithelial cells and express K18. During apoptosis of
carcinoma cells induced by cancer therapy, K18 is cleaved and
subsequently released to the extracellular environment and into the
blood. In contrast, chemotherapy-induced apoptosis of other cell types
such as lymphoid cells will not generate K18 cleavage products.
Caspase-cleaved CK18 is therefore an ideal biomarker for monitoring
apoptosis of carcinoma cells in blood. Recent studies have indeed showed
increased levels of caspase-cleaved K18 (ccK18, K18Asp396-NE or M30
neo-epitope) in sera from cancer patients subjected to different
treatment modalities (Kramer
et al.,
2004; Kramer et al., 2006).
These increases are associated with clinical response to treatment (Hägg
Olofsson et al.,
2007).
Liver disease
Liver cells express K18. During different forms
of liver disease that involve apoptosis, K18 is cleaved resulting in
exposure of the M30-epitope in tissue (Bantel
et al.
2001). Increases in soluble
caspase-cleaved K18-Asp396 fragments have been demonstrated in the
circulation of patients with different types of liver diseases,
including hepatitic C virus infection (Bantel
et al.
2004) and nonalcoholic
steatohepatitis (NASH) (Wieckowska
et al.
2006). The
application of the M30 Apoptosense® assay as a non-invasive marker of
steatohepatitis has large medical applications (see Comment Hepatology.
2007 Feb;45(2):545).
Other Applications
Many other diseases involve cell death by
apoptosis, too. Recent research has shown that infections of hantavirus
lead to increased values in the M30 Apoptosense® ELISA (Klingström
et al.,
2006). Also for sepsis
correlation with this test has been shown (Roth
et al.,
2004).
The M30 Apoptosense® ELISA has also been used to monitor transplantation
patients (Luft
et al.,
2007) and in clinical trials for
drugs on Human Liver Preservation Injury (Baskin‑Bey
et al.,
2007).
M30 CytoDeath™

The M30 CytoDeath™ ELISA is a novel product developed for cell culture
applications. The assay has a dynamic range and sensitivity suitable for
in vitro work. The M30 CytoDeath™ ELISA is a powerful drug screening tool and is useful
for in vitro characterisation of apoptosis-inducing drugs. The assay
offers a unique possibility to quantify apoptosis of epithelially
derived cells in multicellular spheroids and organ culture systems. The M30 CytoDeath™ ELISA is based on the M30 antibody (detecting the
Asp396 caspase-clevage site on K18) and antibody M6.

The M65® ELISA is intended for the quantitative detection of keratin 18
(K18) present in cell culture supernatants, human serum, human plasma or
other body fluids after it has been released from dying cells into the
extracellular compartment. The M65® ELISA offers the opportunity to
quantify total cell death (due to apoptosis and necrosis) of K18
positive (epithelial) cells.
The M65® ELISA when used in combination with the M30 Apoptosense® assay
(which specifically measures apoptosis) allows to quantify and
distinguish different forms of epithelial cell death as both assays are
standardized against the identical reference (Kramer et al., 2004).

In eukaryotic cells, the cytoskeleton is composed of three different
types of morphologically distinct filamentous structures:
microfilaments, intermediate filaments (IF), and microtubules.
The epithelial keratins (IF types I and II) are conserved
phylogenetically and are closely related, biochemically and
imunologically.
Keratins 1 - 8 constitute the type II group (53 - 68 kDa, neutral to
basic protein components), while keratins 9 - 20 constitute the type I
group (40 - 56 kDa, acidic proteins).
Upon release from dying cells, keratins provide useful markers for
epithelial malignancies, distinctly reflecting ongoing cellular
turnover. Certain keratins, the most prominent example being keratin 18,
are substrates for lethal caspase activation and the subsequent release
of defined protein fragments occurs during apoptotic cell death.
The clinical value of determining different soluble keratin protein
fragments in blood samples (i.e. using the M30 Apoptosense® ELISA or
M65® ELISA) lies in the rapid and non invasive assessment of drug
efficacy or response to therapy in epithelial cell carcinomas.
